The new South Niagara hospital will be located at Biggar Road and Montrose Road in Niagara Falls.
The facility will feature Centres of Excellence in stroke, complex care, seniors’ wellness and academic education with a senior’s focus embedded into all aspects of the patient experience.

Seniors’ Wellness Follow-up Clinic: provide specialized follow up treatment, education and care coordination services for the most complex patients who cannot be followed in the community. This will include coordinated therapy services as well as nursing, nutritional and case management support. The clinic will support patients in the community, including those who have recently been discharged from inpatient services.
Geriatric Rapid Assessment: provide comprehensive evaluation for elderly patients who are referred from the emergency department or from inpatient services.
Geriatric Assessment Clinic: provided for the frail elderly living in the community and experiencing multiple complex problems that cannot be resolved by family physicians/other healthcare providers.
Other Services: The program will continue to work with internal and external partners to host needed services, such as falls clinic and/or caregiver support programming, within available space.

Inpatient Care
Complex Care is a time-limited program for patients 18 years of age and older who require daily skilled assessments and hospital care. Patients will have complex but medically stable clinical conditions. The goal of admission is to enhance the health, functioning and quality of life of the patient to meet specific goals in order to transition back into the community.
